You are listed as a Designated Representative for one of our Members. Each member (receiving CDPAP) must complete the registration with PPL or a PPL Facilitator in order to receive CDPAP services. Everybody receiving CDPAP must be registered with PPL or PPL Facilitator in order to receive CDPAS services on or before 4/1/25.  In Addition, your CDPA (Aide) must also be registered. The information we are tracking below will help us to know who needs to register with PPL or a PPL Facilitator. Each CDPA must register with PPL or a PPL Facilitator!  Please encourage them to register with PPL or a PPL Facilitator by completing 6 steps.

Member / Consumer Registration Process in Four Steps:

  • Step 1: Sign the Consumer Memorandum of Understanding (MOU) – PPL or Facilitator will send you this via a PPL Registration Link to a software called PPL@Home
  • Step 2: If Applicable, Complete the Designated Representative Form (if you have a designated representative)
  • Step 3: Sign the “Consumer and Personal Assistant Agreement” (this is between you, the member, and the PA) – PPL or Facilitator will send them this form
  • Step 4: In order to approve timesheets: Have a Sign In (User Name and Password) for the Time Record System called PPL@Home Time4Care https://pplfirst.com/programs/new-york/ny-consumer-directed-personal-assistance-program-cdpap/

CDPA Registration in Six Steps:

  • Step 1: Sign the “Consumer and Personal Assistant Agreement” – PPL or Facilitator will send them this via a PPL Registration Link to a software called PPL@Home
  • Step 2: Complete 3 Tax Forms:
    • USCIS Form I-9
    • IRS Form W-4
    • NYS Form IT-2104
  • Step 3: Complete 5 Other Forms:
    • Medical Release Form
    • Offer Letter
    • Handbook / Policy Acknowledgement Form
    • Health Assessment Attestation Form (Required if CDPA does not have a copy of the last Health Assessment)
    • Direct Deposit Form (Optional, but great if they want their paycheck quickly)
  • Step 4: Have a Sign In (User Name and Password) for the Time Record System called PPL@Home Time4Care https://pplfirst.com/programs/new-york/ny-consumer-directed-personal-assistance-program-cdpap/
  • Step 5: Complete the PPL@Home training (its free)
  • Step 6: Once all the above complete, Use PPL@Home Time4Care to record time worked. PPL has rolled out a bonus! CDPAs that transition to PPL by 3/28, can sign up for a $100 Bonus – paid by PPL!! And you get paid on time!!
