The Gardens at St. Anthony is a newly renovated building with modern, accessible apartments in Syracuse, NY, available to low-income seniors (age 55+) eligible for Medicaid who are currently homeless or at risk of becoming homeless and meet specific qualifications.

The Gardens at St. Anthony is a beautifully restored and updated low-income senior-living residence on the Southside of Syracuse.

There are 27 one-bedroom units available through Nascentia Health to individuals or couples who meet certain eligibility requirements and work with social services providers.

Qualifications for Tenants

  • Eligible for Medicaid
  • 55 years or older
  • Currently homeless or at risk of becoming homeless
  • Must require assistance with at least one of the following items:
    • Bathing and showering—ability to bathe self and maintain dental, hair, and nail hygiene
    • Continence—lacking complete control of bowels and bladder
    • Dressing—ability to select appropriate clothes and outerwear, and to dress self independently
    • Mobility—being able to walk or transfer from one place to another, specifically in and out of a bed or chair
    • Feeding (excluding meal preparation) —the ability to get food from plate to mouth, and to chew and swallow
    • Toileting—ability to get on and off the toilet and clean self without assistance
    • Managing medications and taking medicines as directed
    • Cleaning and housekeeping, including maintenance and other home care chore
    • Doing laundry
    • Managing money
    • Preparing meals
    • Shopping for groceries and other necessities
    • Transportation, including changing residences and moving
    • Using communication devices, including a telephone or computer
