Nascentia Health’s Empire State Supportive Housing Initiative (ESSHI) provides permanent housing and support services for adults 55+ who are experiencing homelessness as well as health or social challenges.
The ESSHI Program provides a stable home first, then connects senior to the support they need to live independently.

What We Provide
- 27 one-bedroom accessible apartments with a full kitchen, living room, bedroom, and full bathroom
- Affordable housing for low-income seniors in a restored historic building on Syracuse’s South Side
- Support services such as healthcare coordination, life skills training, social activities, transportation, and more
Who May Qualify
Applications must meet the following qualifications:
- Homeless and over the age of 55
- Currently enrolled in Medicaid
- Must have a chronic condition or physical disability
