About Medicare and Medicare Advantage
Comprehensive and Personalized Care in Long-Term Care Facilities
Each member is paired with a clinical care provider, such as a nurse practitioner or physician assistant, who works closely with their primary care physician and long-term care facility staff. The clinical care provider oversees routine and as-needed exams, communicates with doctors, staff, and family members, and creates a care plan tailored to each member’s health needs. This collaborative approach help ensures coordinated care for each member.

Vision Upgrade
$700 each year
Visit your eye doctor annually and use your upgrade toward frames, lenses, and contact lenses.
Dental Care
$3,000 Dental Coverage, Covered with prior authorization
Receive comprehensive and preventive care, exams with cleanings, and annual x-rays.
Quarterly Beauty Benefit
Enjoy visits to the in-house beautician throughout the quarter to keep you looking and feeling your best.
Companion Care
200 hours per year for eligible members
For eligible members, non-skilled companions support dementia care, safety, function, and social connection.
Eligibility:
- Enrolled in Medicare Parts A and B
- Continue to pay your Medicare Part B premium, if it’s not paid by Medicaid or another payer
- Must require or are expected to need the level of services provided in a long-term care skilled nursing facility for 90 days or longer
- Must be a resident of one of our participating nursing home partners:
- Roscoe Rehabilitation and Nursing Center
- Service area, benefit limitations, and restrictions might apply
