Nascentia Health Plus is a Medicare Advantage Dual Special Needs Plan that gives extra benefits to people on both Medicare and Medicaid.

Medicare Part B premium, if applicable
Key Plan Benefits
Food, OTC, & Utility Bills – $225 per month
Buy healthy groceries like produce, dairy, bread, and meat; and OTC items such as cough and allergy medications, first aid supplies, and more at your favorite stores. Use our monthly benefit for utilities such as heat, electric, and internet.
Hearing Aids
$2,000 each year
Get new hearing aids for both ears annually to make sure you can hear clearly.
*$1,000 per ear
Vision Upgrade
$350 each year
Visit your eye doctor annually and use your upgrade toward frames, lenses, and contact lenses.
Dental Care
Covered with prior authorization
Receive comprehensive and preventive care, two oral exams with cleanings, and annual x-rays.
Fitness Pass
Unlimited Use
The One Pass fitness program includes hundreds of gyms and online health and wellness classes. Learn more about One Pass.
Social & Cognitive Wellness Program
This program supports eligible chronically ill DSNP members through in-home technology that promotes social engagement, cognitive stimulation, health education, and wellness.
Eligibility:
To be eligible for the Nascentia Health Plus Dual Special Needs Plan:
- Enrolled in Medicare Parts A and B
- Eligible for Medicaid benefits
- Continue to pay your Medicare Part B premium, if it’s not paid by Medicaid or another payer
- Must live in our 15-county service area:
- Albany, Broome, Columbia, Delaware, Erie, Greene, Madison, Monroe, Niagara, Oneida, Onondaga, Otsego, Rensselaer, Saratoga, and Schenectady
