Comprehensive Care From A Single Trusted Plan
Our plan matches each member with a clinical care provider, such as a nurse practitioner or physician assistant, who will work with you in addition to your primary care physician (PCP).
The clinical care provider is responsible for overseeing, facilitating, and coordinating your health care needs and performing routine and as needed physical examinations, depending on your health care status.
Your clinical care provider will work closely with your PCP and your nursing home staff to develop an individualized care plan to ensure your specific health care needs are met. The clinical care provider will promote collaboration among your physicians, family, and the Nascentia Skilled Nursing Facility insurance plan.

*Member must continue to pay their Medicare Part B premium
Over-The-Counter - $45 per month
Buy OTC items such as cough and allergy medications, first aid supplies, and more.
- View included food and OTC items
Hearing Aids - $4,000 each year
Get new hearing aids for both ears annually to make sure you can hear clearly.
Dental Care - $4,500 each year
Receive comprehensive and preventive care, two oral exams with cleanings, and annual x-rays.
Vision Upgrade - $700 each year
Visit your eye doctor annually and use your upgrade toward frames, lenses, and contact lenses.
Transportation - 48 each year
Get to important appointments with 48 medically approved one-way trips each year.

Eligibility:
- Enrolled in Medicare Parts A and B
- Continue to pay your Medicare Part B premium, if it’s not paid by Medicaid or another payer
- Must require or are expected to need the level of services provided in a long-term care skilled nursing facility for 90 days or longer
- Must be a resident of one of our participating nursing home partners:
- Loretto Health & Rehabilitation
- Service area, benefit limitations, and restrictions might apply
