42 health-related meals following surgery or hospitalization, $0 Annual Premium* and Deductible, Part D Prescription Drug Coverage, and $0 Preventative Care and Screenings

Food & Over-The-Counter - $200 per month
Buy healthy groceries like produce, dairy, bread, and meat; and OTC items such as
cough and allergy medications, first aid supplies, and more at your favorite stores.

Utility Bills - $100 per month
Use our monthly benefit for utilities such as heat, electric, and internet to help pay for these essential services.
Annual Rewards - $175 each year
Earn rewards when you get preventive care like yearly doctor, eye, and dental exams, and a flu shot.
Hearing Aids - $2,500 each year
Get new hearing aids for both ears annually to make sure you can hear clearly.
Dental Care - $2,000 each year
Receive comprehensive and preventive care, two oral exams with cleanings, and annual x-rays.
Vision Upgrade - $600 each year
Visit your eye doctor annually and use your upgrade toward frames, lenses, and contact lenses.
Fitness Pass - Unlimited use
The One Pass fitness program includes hundreds of gyms and online health and wellness classes.

Eligibility:
- Enrolled in Medicare Parts A and B
- Eligible for Medicaid Benefits
- Continue to pay your Medicare Part B premium, if it’s not paid by Medicaid or another payer
- Must live in our 15-county service area:
- Albany, Broome, Columbia, Delaware, Erie, Greene, Madison, Monroe, Niagara, Oneida, Onondaga, Otsego, Rensselaer, Saratoga, and Schenectady
